Non-narcotic adjuvants may improve the duration and quality of analgesia after knee arthroscopy: a brief review

Objectif : Nous avons évalué les études randomisées et contrôlées portant sur l’administration intra-articulaire de clonidine, néostigmine, anti-inflammatoires stéroïdiens et non stéroïdiens comme adjuvants aux anesthésiques locaux après une arthroscopie du genou.

Méthode : Vingt-cinq études ont été revues selon des critères spécifiques d’inclusion et, selon le cas, retenues pour l’évaluation finale. Nous avons évalué les scores de douleur, la consommation totale d’analgésique et le temps écoulé avant la première demande d’analgésique afin de déterminer un effet périphérique possible, en opposition aux effets généraux possibles d’un adjuvant administration dans l’articulation.

Résultats : Seize études répondaient aux critères d’inclusion. On y utilisait des anti-inflammatoires non stéroïdiens, des stéroïdes, de la néostigmine et de la clonidine. Le kétorolac, dans quatre études (pour un total de 230 patients), montrait une amélioration significative de l’analgésie. La clonidine affichait un effet analgésique considérable et des effets indésirables minimes dans les sept études évaluées (pour un total de 424 patients). En outre, nous avons trouvé que l’addition de clonidine à la bupivacaïne ou à la morphine augmentait la durée et la qualité de l’analgésie postopératoire.

Conclusion : Une variété de médicaments a des effets synergiques quand ils sont ajoutés aux anesthésiques locaux et il y a des indications d’une amélioration de l’analgésie, au moins partiellement, selon un mécanisme local plutôt que central. Les résultats de cette revue indiquent que la clonidine et le kétorolac, administrés dans l’articulation après l’arthroscopie du genou, peuvent réduire la douleur postopératoire.
