General Information
In 2008, subscribers of the Canadian Journal of Anesthesia: will receive:
The Canadian Journal of Anesthesia publishes twelve issues per year plus an electronic edition of
the Annual Meeting Supplement (June 2008).
All subscriptions commence with the January issue regardless of when payment is received.
Back issues will be sent to the subscriber to ensure receipt of a complete volume.
Discounted multiple-year rates are not available and subscriptions must be renewed annually.
Renewal notices are mailed in the Fall each year.
Payments
To facilitate payment processing, please include the bottom portion of the invoice with your payment.
Cheques must be made payable to the Canadian Journal of Anesthesia.
Cheques must be payable in Canadian or equivalent US funds and must be drawn on a Canadian or US bank.
Only Canadian or US dollars will be accepted. All other
currencies will be returned.
Credit card payments are accepted (Visa, MasterCard and American Express).
Please include name
of card-holder, expiry date and authorizing signature.
Cancellations
Cancellations will be accepted, however, refunds will be pro-rated. Please allow 6 weeks for processing.
Missing issue claims
Canadian and US subscriptions: a missing issue claimed within 3 months of its mailing date will be replaced free of charge.
After this period, a replacement copy must be purchased.
Overseas subscriptions: a missing issue claimed within 6 months of its mailing date will be replaced free of charge.
After this period, a replacement copy must be purchased.
Damaged or defective copies will be replaced free of charge providing the original copy is returned.
Claims will not be filled under the following circumstances:
Failure of address change notification (we require a minimum of 6 weeks notice for an address change).
Inadequate mail service. If we are able to verify that an issue was mailed, we cannot be responsible for its non-delivery.
Back issues
Single back issue: $25.00
Supplements: $15.00 (includes Annual Meeting and industry supplements, as applicable)
Please include $ 5.00 per issue for surface mailing or $ 10.00 per issue for airmail delivery.
Full payment is required in advance.
Please note that on Canadian orders 5% GST (or 13% HST) is applicable to the total amount (back issue cost plus postage).
